Founder and Provider
Hi, I’m Sydney, a Board-Certified Psychiatric Mental Health Nurse Practitioner (PMHNP). I know that reaching out for mental health support can feel overwhelming, and I want you to know you don’t have to do it alone. My goal is to create a safe, welcoming space where you feel heard, supported, and understood.
I work with adults aged 18 and up, taking a collaborative approach—meaning we’ll work together to find what feels right for you. That may include medication, coping strategies, or lifestyle changes, but most importantly, it’s about creating a plan that fits your life and your goals.
I earned my Master of Science in Nursing from The University of Saint Joseph, completing training in outpatient and inpatient psychiatry as well as community mental health. Over the years, I’ve had the privilege of working with individuals managing addiction, anxiety, depression, ADHD, mood disorders, trauma, and other mental health challenges.
At Summit Mind and Wellness, my practice is compassionate, patient-centered, and integrative. I draw on a college minor in Nutrition to incorporate dietary and lifestyle guidance into care, and I am certified in Acudetox, a specialized acupuncture approach that targets ear points to help reduce cravings, support addiction recovery, and promote emotional balance. By combining traditional psychiatric care with these holistic tools, I help patients support their mind, body, and overall well-being.
Outside of my practice, I’m a wife and mom, and I know firsthand how busy, messy, and beautiful life can be. I bring that perspective into my work, meeting you where you are and walking alongside you as you move toward feeling better.

Standard/Follow-up Visit $125
*Price reflects self-pay rate, prior to any insurance coverage or reimbursement.
*A superbill can be provided for out-of-network services.
Good Faith Estimate
Under the law, health care providers are required to give patients who don’t have insurance, or who are not using insurance, an estimate of the expected charges for medical services. This is called a Good Faith Estimate (GFE).
You have the right to:
Receive a GFE in writing at least 1 business day before your scheduled service.
Ask your provider for a GFE before you schedule or at any time.
Dispute charges if your bill is at least $400 more than your GFE.
The Good Faith Estimate will include expected costs of services, such as session fees, and is meant to help you plan for your care. Please note, the GFE is only an estimate and final charges may vary depending on your needs.

Policy Summary
Appointments & Cancellations
Appointments must be canceled at least 48 hours in advance.
A $75 fee may apply for late cancellations or missed appointments (subject to insurance regulations, including Medicaid restrictions).
Repeated missed appointments may result in discharge from the practice.
Services are available in person or via telehealth depending on clinical appropriateness.
Payment
Payment is due at the time of service unless otherwise arranged through insurance.
Patients are responsible for any costs not covered by insurance.
A card on file may be requested for convenience but is not required when prohibited by insurance regulations.
Medication Management & Refills
Refills require at least 3 business days’ notice and are processed during business hours only.
Ongoing prescriptions require regular follow-up appointments.
Controlled substances are prescribed with careful monitoring and may include:
Review of prescription monitoring programs
Urine drug screening (when clinically indicated)
Medication counts
Coordination with other providers
Misuse, diversion, or inconsistent monitoring may result in medication discontinuation.
Adolescents (Age 16+)
Patients age 16 and older may be accepted on a case-by-case basis.
Parents/guardians may participate in evaluations, but adolescents will have private 1:1 time with the clinician as part of care.
Forms & Paperwork
Disability, FMLA, or extensive paperwork generally requires at least 3 months of established treatment.
Fees may apply depending on complexity.
Communication
Messages are typically returned within 2 business days.
This practice does not provide emergency or crisis services.
Emergencies
In an emergency, call 911 or go to the nearest emergency department.
You may also call or text 988 for the Suicide & Crisis Lifeline.
Professional Conduct
Respectful communication is expected.
Threatening, abusive, or unsafe behavior may result in immediate termination of services.
Technology & Documentation
Technology tools, including AI-assisted documentation, may be used to support clinical care.
All records are reviewed and finalized by the clinician and remain confidential per HIPAA regulations.
